Relay deployment test

This page confirms that the default index is replaced. Use the health check to confirm the app runtime is attached, then use the relay test to call an allowed bank URL through the server.

Expected result

If `/health` returns JSON, Plesk is routing requests to the relay app. If this page opens but `/health` returns a Plesk 404, static hosting is working but the Node.js or .NET app is not enabled correctly.
